Each of our hospice team members plays an important role in every patient and family we serve. Aside from being licensed, insured, and bonded, our hospice team is driven and passionate to provide the care each of them deserves. Our hospice staff consists of the following:

Primary Care Nurse: A primary care nurse handles various nursing needs and coordinates with the patient’s family members, physicians, and other health care providers for the best care plan possible. Our nurses are responsible for preventing and relieving pain and other symptoms each patient feels. They also educate the family members of each patient on how to properly care for their ill loved ones and more.

Physician Services: Along with the patient’s attending physician, we have hospice medical director and physician employees who will help them manage their condition and other disease-related problems.

Social Worker: Our social workers will provide patients and their family members with emotional support, counseling, and guidance throughout the end-of-life process. Other than that, they also help them find all the necessary resources available to help them cope with problems that may arise. They will also lend a hand in legal, insurance, funeral arrangements, and other matters that they need help in the process of coping.

Physical, Occupational and Speech Therapists: We have licensed therapists who provide therapy services to patients if only deemed reasonable and necessary as per the patient’s physician’s advice.

Volunteers: We have highly-skilled and well-rounded volunteers who are good listeners, empathetic, nonjudgmental, adaptable, and compassionate to the patients and their family members.

Dietary Counselor: To address and ensure that the patient’s dietary needs are met, our dietitian or nurse can provide dietary counseling to the patients and their family members.

Chaplain: Meeting all patients spiritual needs.

Home Health Aide: Our Home Health Aides are there to assist and help with ADLs and also in helping with the basic needs of the patient and family.
