Five Star is a hospice and palliative care provider based in Hoffman Estate, Illinois. We know that the final stages of life bring about complex challenges not only for the patients but to their family members as well. That is why we are here to assist our patients throughout their remaining days, and their family members too, giving them the support they need to cope with this difficult time in their lives.
